JOHN MILLEDGE ACADEMY
REQUIREMENTS FOR REGISTRATION

2008-2009 School Year

1. A certified copy of the birth certificate
2. Social Security Number
3. Current immunization form #3231
4. All new students must have an ear, eye, and dental certificate #3300
5. All students must have documentation of the chicken pox vacine or documentation that your child has had chicken pox.
6. All former school records and test scores (if applicable)
7. Registration and Book fees paid
8. All students registering for grades 7 through 12 must have a "Letter of Good Standing" from the last school they attended

PLEASE NOTE:

Registering your child does NOT guarantee placement. Acceptance into John Milledge Academy is based on prior grades, test scores, etc. A "Letter of Good Standing" must be sent from the school(s) previously attended.
